Individualized Treatment Plan
Each skilled nursing resident will receive an individualized treatment plan designed for optimum recovery. Goals are set for each resident by an interdisciplinary team comprised of the resident and/or family members, the participant’s physician and our health-care professionals. Weekly team meetings are held to review the progress of the resident and re-evaluate the treatment plan.Once the goals have been achieved or the team determines the resident is no longer progressing through the treatment program, he/she will be discharged home or to an environment that will best meet their needs.
